How do you know what level of care best suits my loved one?

The personal attention and time spent with our intake department is critical. This is when a lot of information is gathered about your loved one's physical, mental and emotional needs as well as medical information. All this information will assist the nurse to help guide you to the proper level of care choice.

We will review your choices with you and then proceed to find the placement that best suits your loved one's needs.

Have you seen the facility first hand?

First Choice is affiliated with Companion Hospice and Companion Home Health. This means that we benefit from the experiences of over 50 nurses being out in the community everyday. We are very well acquainted with the facilities in your area.

In addition, First Choice makes an onsite visit to any facility prior to recommending it to you. We evaluate all aspects of the care given as well as the services offered.

How do I know what I can afford?

During our personal intake process, we will conduct a financial evaluation. This may be a bit uncomfortable but is necessary.

We are committed to providing you with the best facility that meets all of your needs.

My loved one does not speak English. Can you find placement?

First Choice has over 150 facilities that we work with. Many specialize in specific cultural and language needs.

What if my family member requires daily injections?

First Choice works with over 150 facilities. We keep detailed information regarding specialized programs available at each facility. These include:

  • care for memory impairment and wandering
  • daily injections
  • wounds
  • incontinence care
  • assistance with medication
  • fall risk

and many other special needs.

How much does First Choice service cost?
